Your physician or UTMB Health — John Sealy Hospital discharge planner sends a referral to Signature Health Services. We also accept self-referrals and family referrals.
Care Plan
Our team reviews your medical history and discharge orders to create a personalized home health care plan — often starting the same day or next day.
Home Visits
A dedicated clinician from our Galveston County team visits your home on a regular schedule, coordinating with your physician and UTMB Health — John Sealy Hospital as needed.
