Working with physicians, therapists, and community agencies to ensure all aspects of a patient's care are aligned and no needs fall through the cracks.

Helping family members understand the patient's condition, manage caregiver stress, communicate effectively with each other, and plan for short- and long-term care needs.

Collaborating with hospital discharge planners to ensure a smooth transition home — arranging equipment, scheduling follow-up visits, and connecting families with support services.
