Our daily living pathway combines hands-on personal care from certified nurses' aides with skills training from occupational therapists and the resource navigation that medical social workers provide. For many patients and families, this combination is what makes the difference between staying home and needing a facility. We also support caregivers — because the person helping a loved one at home needs guidance, respite, and reassurance too.
